Laryngoscope blades in different sizes and shapes should be available before induction of anesthesia. Laryngoscope blades fall into two categories: straight and curved. Because the epiglottis is angled posteriorly, visualization of the glottis may be difficult. Straight laryngoscope blades are often recommended for use in neonates and infants to lift the epiglottis. The most common straight blades include the Miller, Wisconsin, Wis-Hipple, and Wis-Foregger blades.

Laryngoscopes
The ability to use standard straight and curved laryngoscope blades is mandatory in pediatric anesthesia. The laryngeal structures of infants are encountered at a more cephalad and anterior position, approximately the level of C2 to C3, depending on age, until the child reaches school age, when the laryngeal structures descend to a level opposite the body of C4. It is common practice to use a straight laryngoscope blade such as the Miller 0 or 1, or the Wis-Hippel 1.5, to intubate the trachea of an infant or a child up to school age. A curved laryngoscope blade with a wider spatula, such as the Macintosh blade may be advantageous to control the tongue of an older child. Whether to use the straight or the curved laryngoscope blades can be determined according to this approximate age distribution; however, the individual preferences of the clinician may vary, as can individual circumstances.

| Greater than 50 types of curved and straight laryngoscope blades are available, the most commonly used being the curved Macintosh blade.20 Using specific blades in certain circumstances has been both encouraged117–119 and discouraged.120 In patients with a large lower jaw or “deep pharynx,” the view at laryngoscopy is often improved significantly, by using a size 4 Macintosh blade (rather than the more common adult size 3). This ensures the tip of the blade can reach the base of the vallecula to lift the epiglottis. Other blades, such as the McCoy, may be advantageous in specific situations.121,122 |
